Enrolling With Us

Thank you for choosing to enrol with Cashmere Health Practice. We want to make the enrolment process as simple as possible. Please take a moment to read through the following information. If you have any questions, please call our reception team to discuss.

Enrolment Checklist:

1. We are a zoned practice, please phone our reception team to check if you live in our zone.
2. Check your eligibility by clicking here, or for more information, visit Te Whatu Ora by clicking here.
3. Read our Terms of Trade.
4. Download and complete the Enrolment form. Please make sure to read the Enrolment with General Practice Guidelines also.
5. Every patient over the age of 16 needs to sign their own enrolment form. Please bring completed forms along with photographic ID into the practice. Please do not email completed forms as original ID must be seen by our staff in person.
6. If you are an NZ citizen, please bring one type of photographic ID such as a driver’s license, student ID, Kiwi Access Card, or passport.
7. Other nationalities will be required to present their passport and visa information as per the Ministry of Health requirements. More information around visa’s can be found by clicking here.
