Enrolling with Us

Guide to Patient Enrolment

Thank you for your interest in enrolling with Cashmere Health. Please read through the following information. If you have any questions please call our reception team.

In order to be an enrolled patient with us, you need to live within our boundaries and do the following:

  1. Download and complete the Enrolment form.
  2. Complete the form in full.
    We require one enrolment form per person that is enrolling with us.
    For children under 16 years old (including babies), their parent or guardian needs to complete the form and sign on their behalf. Every person over the age of 16 years old, must complete and sign their own enrolment form.
  3. Please bring the form into Cashmere Health during our normal opening hours (8:30am to 5pm Monday to Friday) so that our reception staff can check the form and complete the enrolment process with you.

When you bring in the enrolment form into the practice you must also bring in:

  • One form of ID that has a photo on it, such as a driver’s licence or passport.
  • Two items, such as a power bill, phone bill, WINZ letter etc, which confirms your address as the one shown on the enrolment form.
  • ID is a government requirement for anyone enrolling with a new medical centre.
